Case Manager (ACT Team)
Curry Community Health is looking to hire a Case Manager (ACT Team) to work in Curry County, Oregon, located in a beautiful community on the Southern Oregon Coast. This is a full time position offering health benefits (medical, dental, vision), holiday pay, paid time off as well as life insurance and 403b.
The Case Manager will assist in treating individuals with mental, emotional or substance abuse problems. Services provided will be consistent with the Recovery and Strengths-Based models and will be based in a therapeutic relationship. Activities include case management, skills training, client advocacy, prevention and education both 1:1 and in groups.
Some Responsibilities include:
Provide case management services by providing assistance in applying for benefits to which the individual may be entitled.
The case manager will assist the crisis team; this may include contacting possible placements, faxing documents and assisting with discharge planning, in addition to providing assistance to individuals who are in crisis in the community.
When significant health and safety concerns are identified the Case Manager assures that necessary services or actions occur to address the identified health and safety needs of the individual.
Provide skills training services to clients to enhance ability to maintain school or residential placements including activities to improve peer and family relationships.
Work Location: The Case Manager has an office location in Brookings. At times the individual will be called on to serve in multiple locations.
Graduate from an accredited Bachelor Level college program in social work, psychology or related field preferred. A minimum of four years professional experience in one of the above fields in lieu of Bachelor’s degree may be considered.
The individual in this position will maintain a thorough knowledge of community resources and how to enroll individuals in benefits, Medicaid regulations and compliance issues; practices, procedures, and medical record keeping practices.
Effectively uses resources, demonstrates initiative and accepts responsibility.
Possess a current driver’s license, pass a pre-employment drug test and pass a criminal background check required by the State of Oregon.
Pay Range: $18 to $22 per hour
